MDH Survey Reports Nursing Homes and Assisted Living Facilities
Nursing Home Survey Reports for Minnesota Department of Health. This information has recently been updated, and is now available at MDH Office of Health Facility Complaints website.
You can access the information regarding the past two surveys for nursing homes in Minnesota, as well as surveys for Assisting Living, Housing with Services Providers and other elder care housing providers.
This website is not intended to provide legal advice as each situation is different and specific factual information must be obtained before an attorney is able to assess the legal questions relevant to your situation.
If you have concerns about the quality of care provided to residents in a nursing home, assisted living facility, memory care facility, group home or other type of elder care facility or provider you can contact the Minnesota Department of Health and file a request that the concern be investigated by the Minnesota Adult Abuse Reporting Center at 1-844-880-1574.
Common forms of elder neglect and abuse includes sexual abuse, physical abuse, medication errors, fractures and head injuries due to falls from beds, wheelchairs, and lifts. Many accidents happen when a call light is not answered and a resident attempts to transfer themselves.
